Note: Adults must fill out a waiver for themselves individually. Only minors can be grouped into a joined waiver. For families/groups with more than 6 minors, you will need to fill out multiple waivers.
The Following button leads to a printable waiver form intended for daycares/camps only. Anyone who presents this paper waiver that is not at Interskate for a scheduled daycare/camp trip will be asked to fill out an online waiver that will be valid through the end of the calendar year.
